Where do I get the form?
You can download the form by clicking on the link below. It's in PDF so you'll need the Acrobat reader. You can get a free copy of the reader from the Adobe website if you haven't got it already.

What details do I need to put on the form?

You usually only need to fill in:

  • Name of Undertaking e.g. Tesco PLC
  • Description of Security e.g. Ord 25p
  • Total holding in words e.g. 'one hundred'
  • Figures - write the total holding in numbers e.g. 100
  • In the name(s) of - give the name and address of the owner(s) known to the registrar
  • Please sign here - sign the form where indicated.

Please also note:

  • You need to fill in one CREST transfer form for each type of stock holding you're transferring
  • If all certificates are for the same stock, you'll only need to fill in one form. The only exception to this would be if they have different designations. Most holdings will not have any associated designation
  • The certificates must be in your name or your husband's or wife's name. If the certificates are in your husband's or wife's name, he or she must fill in the CREST transfer form and write a letter to confirm that the certificates are being 'gifted' to you, the Lloyds TSB Share Dealing account holder.
