If you’re transferring from another broker to Lloyds TSB Share Dealing, or are transferring any ISAs to us, please fill in the relevant form/s and send them to the address above.

If you deal in US securities you'll need to complete a W-8BEN form, which means you'll pay a reduced rate of US income tax on your investments. Please fill in this form and send it to the address above.

• W-8BEN (certificate of foreign status)

We have forms for a variety of things, such as additional subscriptions to your ISA, opening a joint account or nominating someone to act on your behalf.
